Abstract

|
목적: 본 연구는 적절한 침상안정 시간 규명의 기초자료 마련을 위해 시행된 연구로, 혈액종양내과환자의 골수검사 후 침상안정 시간에 따른 불편감 및 출혈여부, 출혈에 영향을 미치는 요인을 조사하는 연구이다.
방법: 2017년 1월부터 2017년 9월까지 S병원에서 골수검사를 시행한 131명의 환자를 대상으로 자가보고 형식을 이용하여 자료를 수집하였다. 자료의 분석은 SPSS 23.0을 이용하였고, 서술적 통계, Wilcoxon signed-rank test, McNemar’s test 및 로지스틱 회귀분석을 시행하였다.
결과: 골수검사 후 침상안정 2시간 후의 불편감에 비해 4시간 후의 불편감은 유의하게 증가하였다(p<.001). 골수검사 후 침상안정 2시간 후와 4시간 후 출혈여부는 유의한 차이를 보였으나(p<.001), 모든 출혈은 경미한 출혈에 불과했고, 침상안정 2시간 후 거즈를 제거하고도 출혈이 발생하지 않은 환자가 84%로 보고되었다.
결론: 본 연구 결과, 골수검사 후 침상안정 시간을 단축시키는 것이 환자의 안위 증진에 도움이 된다는 것을 알 수 있었으며, 골수검사 부위 및 환자의 상태에 따라 침상안정을 단축시킬 수 있는 것을 확인할 수 있었다.

Purpose: The purpose of this study was to investigate hemato-oncology patients’ discomfort and bleeding in relation to the bedrest time after bone marrow examination.
Methods: A descriptive correlational study was conducted. The data were collected using self- report questionnaire from total of 131 patients who underwent bone marrow examination from January 2017 to September 2017. Data were analyzed with descriptive statistics, Wilcoxon Signed-rank test, McNemar's test and logistic regression.
Results: The level of discomfort after 4 hours of bedrest was significantly higher when compared to 2 hours of bedrest(p<.001). The occurrence of bleeding after 2 hours of bedrest was significantly higher than 4 hours of bedrest(p<.001), however the degree of bleeding was slight. No bleeding occurred in 84% of the patients after 2 hours of bedrest.
Conclusion: The results of this study demonstrated that shortening the bed rest time after bone marrow examination was helpful in improving the patient's well-being. Bedrest time could be shortened according to the site of bone marrow examination and patient's condition.
